Passports

 

In Santa Cruz County, Passports can be obtained by applying in person at various post offices in the county or at the County Clerk's Office. Under some circumstances applications can be submitted by mail. For complete information about passports visit the US State Department web site at http://travel.state.gov/passport_services.html

Application Form: In Santa Cruz County Passport application forms can be obtained at the County Clerk's Office and some Post Offices. The forms can also be obtained through the US State Department web site. That web site also lists other locations that the forms can be obtained.

Completing the Application Form:
·Type or Print in black or blue ink only
·Use complete names (first, middle, last) for everyone
·When applying in person Do Not Sign application or photos.
·Anyone 13 or older must apply in person, unless they qualify for application by mail.

Application by Mail
To submit an application for Passport by Mail, you must answer yes to all of the following:

·I can submit my most recent passport
·I was at least 16 years of old when my most recent passport was issued.
·I was issued my most recent passport less than 12 years ago.
·I use the same name as on my most recent passport, OR I have had my name changed by marriage or court order and can submit proper identification to reflect my name changes.

Proof of Citizenship
1) Previous passport OR a Certified Copy of Birth Certificate issued by the state or county. It must have a State or County seal. HOSPITAL BIRTH CERTIFICATES ARE NOT ACCEPTABLE.
or
2) Naturalization Certificate

Identification
·Current, valid, permanent Driver's License (An expired license or temporary license is NOT ACCEPTABLE.) or Government Identification Card
State Identification Card
·Photographs
·2 identical passport photographs. Photos must look like you at the time you apply and must have white or off-white background.

Passport photographers are listed in the yellow pages of the phone book.

Children
·All Minors must appear in person. A parent must accompany the minor to countersign the application.

Fees
·ADULTS (16 and over): $55.00 CHECK OR MONEY ORDER PAYABLE TO "Passport Services" for each application. Plus, an additional $30 cash or check payable to "County Clerk"

·CHILDREN (15 and under): $40.00 CHECK OR MONEY ORDER PAYABLE TO "Passport Services" for each application. Plus, an additional $30.00 cash or check payable to "County Clerk."

·Expedite Fee: $60.00 additional charge. Please inquire with County Clerk for further information on expedite service.
