Passports

 

 

Every passport applicant must appear in person.

No appointment necessary.

Regular hours are 8 a.m. to 4:30 p.m., Monday - Friday.

Mail goes out daily at 2:30pm.

Closed on these Holidays.

 

Application:

Print or type with black ink only. Do not sign the application until requested to do so by a Passport Acceptance Agent.

Required Documents:

1. Proof of U. S. Citizenship. One of the following:

A. Full name of the applicant.

B. Date and Place of birth.

C. Raised, embossed, impressed or multicolored seal of issuing authority.

D. Registrar's signature, and the date the certificate was filed with the registrar's office (must be within one year of the birth).

2. Photo Identification and copy of front and back, i.e. Current Driver's License (Expired or a temporary license is not acceptable), State or Government ID, Passport. If out-of-state US identification is presented, you are required to present a second form of ID, and a photocopy of the front and back of that ID. It should contain the following information: Your photo, full name, date of birth, and issuance date. If you would like us to make a copy of your ID the fee is $1.00.

3. One Passport Photo. Color photo, 2 x 2 inches in size, taken within the last 6 months, full face, front view, with white or off-white background. Do not wear a uniform, nor a hat or headgear that obscures the hairline.

4. Prior Marriages? If there have been previous marriages, you will need to complete a form providing the following information: former spouse's full name, date and place of birth; citizenship, date of the marriage, date of termination, and reason.

Passport Books

Fees - Applicants Age 16 years and older

There are two separate fees that must be paid:

1. U.S. Dept. of State = $110.00 (Check or Money Order ONLY. No credit/debt cards). If more than one applicant, this fee can be combined on one check. (Individual checks are required for expedited service).

2. County Clerk = $25.00 (Cash, Check or Money Order. No credit/debt cards). If more than one applicant, this fee can be combined on one check. (Individual checks are required for expedited service).

Fees - Applicants Age 15 and younger

There are two separate fees that must be paid:

1. U.S. Dept. of State = $80.00 (Check or Money Order ONLY. No credit/debt cards). If more than one applicant, this fee can be combined on one check. (Individual checks are required for expedited service).

2. County Clerk = $25.00 (Cash, Check or Money Order. No credit/debt cards). If more than one applicant, this fee can be combined on one check. (Individual checks are required for expedited service).

Expedited Service

Plus Express Mail Envelopes

For 3 to 4 week service:
To expedite the processing of your passport there is an additional fee of $60.

· Adult (Age 16 & Older): $110 + $60 = $170, plus $25 to the County Clerk

· Age 15 & Younger: $80 + $60 = $140, plus $25 to the County Clerk

For 2 to 3 week service:
Applicants can pay for express mail envelopes to reduce mailing time. The Express Envelope fee for mailing to the U.S. Dept. of State is $19.99. The return Express Envelope fee is $14.85.

· Adult (Age 16 & Older): $110 + $60 + $14.85= $184.85, plus $25 + $19.99=$44.99 to the County Clerk

· Age 15 & Younger: $80+ $60 + $14.85= $154.85, plus $25 + $19.99=$44.99 to the County Clerk

 

Passport Cards

The U.S. Passport Card can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry and is more convenient and less expensive than a passport book. The passport card cannot be used for international travel by air.

2. County Clerk = $25.00 (cash, or check). If more than one applicant, this fee can be combined for all applicants.

Expedited Service

The U.S. Department of State will expedite the processing time for your passport card for an additional fee of $60. Total fees paid to the U.S. Dept. of State are:

· Adult (Age 16 & Older): $30 + $60 = $90, plus $25 to the County Clerk

· Age 15 & Younger: $15 + $60 = $75, plus $25 to the County Clerk

General Information for Both Passport Books and Cards

 

If the minor only has one parent/guardian, evidence of sole authority to apply for the minor must be submitted with the application in the form of a:

  • U.S. or foreign birth certificate, Consular Report of Birth Abroad, or adoption decree, listing only the applying parent

  • Court order granting sole legal custody to the applying parent (unless child's travel is restricted by that order)

  • Court order specifically permitting applying parent's travel with the child

  • Judicial declaration of incompetence of the non-applying parent

  • Death certificate of the non-applying parent

Processing Time

Standard processing: 4-6 weeks, 2-3 weeks for expedited processing, plus time for mail transport. Times vary depending on workload and are posted at www.travel.state.gov

Traveling within 14 Days?

Commercial Services

There are a number of commercial passport services that, for a fee, will "hand carry" your passport application to a passport processing facility. To find a service check online or in the phone book. The Santa Cruz County Clerk's Office cannot recommend any service.
